Uploading Forms/Documents

Based on the school/organization's setup, the required forms or documents can be uploaded to the athlete's profile. (For instance, a signed physical form from the doctor or medical insurance card). There are two ways to complete this process, both start by clicking on the athlete's profile

Through the Upload Button

Step 1: From the athlete's profile, you'll see an Upload button. Pressing this will take you straight to the upload page.

NOTE: If something requires an upload you will see a little red flag with the name of the document(s) that needs to be uploaded. The name of the document is a link that will take you straight to the upload page and it will have the correct Document Type already selected. All you have to do is choose the file and hit upload.

Step 2: Once at the upload page you will be prompted to Choose File. Select the file of the document you need to upload, it must be in a .PDF, .JPG, .GIF, or .PNG file or it will not allow you to upload.

NOTE: Clicking Choose File on a mobile device or tablet will open up your device's camera, prompting you to either choose a photo from your library or take a new picture. 

Step 3: After the correct file has been chosen, you'll need to select a Document Type. (If you clicked on the flag with the document attached to it this will already be populated for you.) If there are no options in the drop down it is likely that your school/organization does not require you to upload anything.

NOTE: There is also a comment box where any comment can be made about the document that is being uploaded.

Helpful Hint: Each user has the ability to upload each document having a 10MB size limit. In addition, coaches, athletic trainers, and administrators who have system permissions will have the ability to upload documents on behalf of athletes associated with their school or organization.

Step 4: After the document type has been selected and any comments are added, press Upload. Once successfully uploaded it will take you to the Manage Documents tab where you can view the active file. Simply Press Done and you are finished.

Note: You do not want to archive any documents unless the information is out dated. You need all of the current information to be in the Active tab under Manage Documents. 

Through the Manage Documents Button 

Step 1: From the athlete's profile, select Manage Documents which allows relevant forms or documents to upload and be stored in the athlete's profile.

Step 2: Click Upload Document to upload a form or document to your profile.

Step 3: Click Choose File to select your document from your smart phone, tablet, or computer.  (The document you select to upload must be in a .PDF, .JPG, .GIF, or .PNG format or it will not upload.)

Step 4: Click Select Document Type, and select the name of the document type you are uploading from the drop down list. Also, you have the ability to add an optional comment to provide additional information to identify your document.

NOTE: If there are no document types in the drop down list, most likely your school or organization is not uploading any forms or documents to the athletes' profiles.

Step 5: When you are ready to upload your document click Upload.

After uploading the document, you will be directed back to the Manage Documents page and the document will be successfully uploaded in the Active section. You can view the document by clicking its name or you can simply hit Done at the bottom of the page to go back to the students profile.
